I am frequently asked by patients whether I recommend breast implants or fat grafting to the breasts to achieve their breast augmentation goals. In the case of most cosmetic breast augmentations I have found that answer almost always to be breast augmentation with silicone breast implants or saline breast implants, whereas in a breast augmentation after breast reconstruction for cancer fat grafting is often all that is needed

The decision to get breast implants or undergo fat grafting to the breasts (or any cosmetic surgery for that matter) is a personal one that depends on your individual goals. Both procedures can be used to enhance the size and shape of the breasts, but they differ in how they are performed and the results they can achieve.

Breast implants are silicone or saline-filled devices that are inserted above or below the pectoralis muscle to increase your breast size and improve your breast shape. These are a popular option for women who want to significantly increase the size of their breasts or who have lost breast volume due to breastfeeding, or weight loss. Breast implants come in a variety of shapes, sizes, and textures, and can be customized to achieve your desired look. Your surgeon will have more control over the shape and size of the breasts.

Fat grafting to the breasts, also known as fat transfer breast augmentation, involves removing fat from one area of the body (such as the abdomen or thighs) using liposuction, and injecting it into the breasts to increase their size and fullness. This procedure can result in natural-looking and feeling breasts but there are limitations on how much fat will survive after the transplantation, typically only 30-50% of the transferred fat cells survive. Many patients require repeat procedures, and implants are better in giving you a symmetrical appearance after just one operation.

Finally, it seems that the statistics bear out my preferences. Year after year, data by the American Society of Plastic Surgeons indicate that breast implants outnumber fat injections to the breast by about 50:1, year after year.
